    • Not only are we caregivers to the patient, we are also there for the family and loved ones. Grace Hospice social workers are an integral part of our team. They are professionally trained in helping patients and their family with the social, psychological, financial and family problems that can occur when a terminal illness is diagnosed.   • Counseling for personal and financial issues

  • Linkage to our Extended Care Facilities and private duty
